The three forms, set up for you
You should not have to learn a form builder to put your enrollment form online. These three come set up for your center, built from whatever you use now: the paper one, a PDF, a photo of it.
- Enrollment
- The one that starts everything. Student details, contacts, and the days they will attend.
- Schedule change
- Which days they attend now, and which days they want instead.
- Pause or cancellation
- Stopping for a month or leaving altogether, with the reason and the dates.
And you can change it yourself
Once a form exists you are not locked out of it. The editor shows the form exactly as a parent sees it, and you edit the questions on that page — there is no separate preview to check against.
- Add or reword a question
- Click it on the form itself and change it.
- Choose the answer type
- Short text, long text, a date, a dropdown, a checkbox, a consent line.
- Mark what is required
- And say so once at the top, instead of a column of asterisks.
One link, one inbox
A published form has a single link you can put in an email, a message or on your website. Everything that comes back arrives in one place, in order, encrypted.

- A closed form shows parents a written notice, not a dead page.
- Tags and internal notes
- Mark what has been dealt with and leave a note for whoever picks it up next.
- Encrypted at rest
- Answers are encrypted in the database, and opening one is recorded against your name.

What is included, and what is not
You get three forms: enrollment, schedule change, and pause or cancellation. Any form beyond those three is $5 a month, in English, and we build it — if you also own the Language Pack it is written in your languages at no extra charge. Two other things sit outside this add-on: it does not email a copy to the parent, and it does not collect a signature.

One price a month, on top of a $19/month platform fee that covers your account whichever add-ons you take. It does not change with how many staff you have or how many students you enroll.

E-signature is an optional upgrade on top of this, and the non-English languages are a separate add-on. Both are genuinely optional — plenty of centers need neither — which is why they are priced separately instead of being used to justify a bigger plan.
